Certification
I have read and agree to observe and abide by the requirements set out in the above documents, in particular in respect of Activities M and M1, the requirements relating to the protection of children from harm: to refuse sales of alcohol to underage persons, persons considered drunk or persons that are purchasing alcohol for another person considered drunk, and to ensure that there is no drunken or disorderly behaviour. No alcohol shall be consumed by those aged under 18, unless they are eating a table meal, in which case no alcohol shall be consumed by those under 16.
I will assume responsibility for so doing throughout the event(s). In the case of Activities M and M1 the following persons will be responsible for supervising the bar. They must at all times ensure that the provisions of the Operating Schedule are adhered to. No other person will be permitted to sell alcohol. The authority for these persons to supervise the bar must be given by a Personal Licence Holder in writing before the event.

There shall be no sale or supply of alcohol when there is no Designated Premises Supervisor (DPS) in respect of this premises licence or at a time when the said Premises Supervisor does not hold a personal licence or when his/her licence is suspended.
Every supply of alcohol under the premises licence must be made or authorised by a person who holds a personal licence.
Any person used to carry out a security activity must be licensed by the Security Industry Authority.
Admission of children (under the age of 18) to any exhibition of films must be restricted in accordance with the film classification body designated as the authority under Section 4 of the Video Recordings Act 1984.
Where the film classification is not specified or the relevant licensing authority has notified the premises licence holder under Section 20 (3)(b) of the Licensing Act 2003, the admission of children must be restricted in accordance with any recommendation made by the Licensing Authority.
Contact telephone numbers for committee members will be available on the premises and displayed in such a manner that they can be read from outside, these will also be available on the Village website and on any distributed information such as the booking form.
The resident caretaker shall normally be present during licensable events and shall be a personal licence holder.
When alcohol is being sold on the premises a member of the committee or someone appointed by the committee who is a personal licence holder shall be present.
All hirers will be provided with a copy of the operating schedule and licensing conditions and have their attention drawn to any areas that specifically affect their function.
The committee shall ensure that:
A Fire and Electrical Safety Risk Assessment shall be completed every 5 years (or less if required by legislation) and the results recorded.
All electrical supplies are covered by Residual Current Devices (RCDs).
hirers shall keep shut all external doors and windows during events involving amplified music or speech.
hirers shall ask patrons to leave quietly and quickly.
hirers shall not erect decorations of a combustible nature.
hirers do not use unauthorised heating appliances.
hirers do not use portable Liquid Petroleum Gas (LPG) appliances.
when alcohol is sold signage will be displayed to indicate that it is the right of bar staff to insist on identification and to refuse sales to underage persons, person considered drunk or persons that are purchasing alcohol for underage persons or person considered drunk
Only a valid Passport, Photo card Driving Licence or ID carrying the PASS logo will be accepted as proof of age.
PEL Embedded Restrictions
There shall be no admissions to Disco type events after 22:30.
Indoor sporting events shall exclude promotion or professional boxing.
Attendants
Throughout the time when the public are present, there shall be no less than one attendant per 250 persons or part of that number.
If the persons present are predominantly children, there shall be at least two attendants per 100 persons or part that number
If the number of persons present on any floor or tier above or below ground level does not exceed 100, there shall be at least one attendant on duty on each floor unless the persons are predominantly children when there shall be at least two.
If the persons present on any floor or tier exceeds 100 there shall be at least two attendants on duty.
If the persons present are predominantly children and are situated on any floor or tier above or below ground level, and the number exceeds 100, there shall be at least four attendants per 100 persons or part of that number.
The licensee, cashier or similar staff shall not be counted as attendants, nor shall doormen, except those attending a door leading directly into the auditorium.
The attendants referred to above, shall be trained in the action to be taken in the event of a fire or other emergency.
All attendants shall wear either distinctive clothing or suitable armbands.
Marking of Doors
The word 'PRIVATE' or the description of the room to which the door leads shall be painted in letters not less than 25mm in height on any door which does not lead to an exit.
Smoking
Smoking will be prohibited within the stage area and dressing rooms and notices should be prominently displayed to this effect.
Hypnotism
No person shall at the premises (other than as provided by section 5 of the Hypnotism Act 1952) give any exhibitions, demonstrations or performance of hypnotism, mesmerism, or any similar act or process which produces or is intended produce in any other person any form of induced sleep or trance in which the susceptibility of the mind to suggestion or direction is increased or intended to be increased.
Maximum Number of Persons
On all occasions when the premises is used for public entertainment the number of persons admitted shall be no more than 280.
Striptease Entertainment
Only activities which have been agreed in writing by Exeter City Council shall be in place.
Striptease entertainment may only take place between the hours of 12 Noon and 12 Midnight on Mondays to Saturdays inclusive.
The agreed activities will only take place in designated areas approved by Exeter City Council. These areas will be in full view of the audience. No part of the premises accessible to the public shall be concealed form observation without written consent of the Council.
Dressing room(s) must be provided and shall not be accessible to members of the audience prior to, during or after striptease.
The striptease entertainment shall be given only by the performers and no audience participation shall be allowed or permitted.
Activities involving one or more performers in close physical contact with either another performer or member(s) of the audience, are not permitted, either in the designated area or any other location in the premises.
Whilst striptease entertainment is taking place no person under the age of 18 years shall be in the licensed premises and a clear notice shall be displayed at each entrance to the premises in a prominent position so that it can be easily read by persons entering the premises on the following terms:
'NO PERSON UNDER THE AGE OF 18 YEARS WILL BE ADMITTED'
Dancers not performing must not be in the licensed area in a state of undress.
Dancers shall not be aged less than 18 years and shall only perform on a stage or to seated customers.
The performers shall at all times wear, for females, a G-String and for males appropriate clothing to cover their genitalia adequately. The clothing in either case shall not be transparent or removed. At no time will performers reveal their genitaila or anus.
There shall be no physical contact between customers before, during or after the performance other than the placing of notes, by the customer, in a garter worn by females and an armband worn by males. Notices outlining this shall be clearly displayed at every table, at each entrance to the premises and each bar.
There shall be at least one Licensee present on the premises during the entertainment to ensure compliance with these conditions. In addition there shall be a nominated female present to oversee the activities of female performers, and likewise a nominated male present to oversee the activities of male performers.
A register shall be maintained and kept on the premises to clearly record the identity of the Licensee(s) on duty, the day and times of the start and finish of the duty and the records shall be retained for a period of not less than 12 months after the last entry in the register. The register shall be available at all times for inspection by the Police or Officers of the Licensing Authority.
CCTV shall be installed, operated and maintained to the satisfaction of the Police to monitor and record activities within all parts of the premises where the performance is taking place.
All video recordings shall be securely stored by the Licensee for a period of not less than 4 weeks following the date of recording. The Licensee shall not allow any recording so made to be viewed by any person except by Police or Officers of the Licensing Authority in association with the Licensee who will be permitted to make copies and produce images for evidential purposes under P.A.C.E.
The Licensee shall not allow or permit the display outside the premises, any photographs or other images which indicate or suggest that striptease or similar dancing takes place on the premises.
A1 The management committee will, through a written hiring agreement, require all hirers holding licensable activities to comply with the safety points relating to the management of premises. The committee will, as managers of the premises, endeavour to provide premises which are safe for public use (where the committee is organising a function itself it will also comply with the points hirers would usually be expected to comply with). The Conditions for Hire and Hall Manager will draw the attention of hirers to these conditions and acquaint them with the location and use of the Hall's emergency equipment, evacuation procedure and “In Case of Fire” instructions. The Hall Manager will also draw the attention of hirers to the need to avoid the creation of public nuisance or disorderly and drunken behaviour.
A2 Contact telephone numbers for committee members will be available on the premises in case of difficulty. They will also be displayed in such a manner that they can be read from outside of the premises. In addition, these details will be held on the Village Web Site and on any distributed information such as the Booking Form.
A3. The Resident Caretaker will normally be present during all licensable events. He will be a Personal Licence Holder.
B1. All Hirers will be provided with a copy of the Operating Schedule and Licensing Conditions and have their attention drawn to any areas that specifically affect their function.
B2. The Hall is not fully open to the Public and attendance is normally only at pre-booked events. The Management Committee will ensure that Hirers are made fully aware of their responsibility to ensure there is no drunken or disorderly behaviour. The Committee have the ability to insist on deposits and use this to emphasize the need for Hirers to operate strict control. The Management Committee reserve the right to attend any function.
B3. Where Hirers are intending to employ door supervisors we will require that they are correctly registered with the SIA, display the correct name badge, if more that one, ensure that one is female. Door supervisors will ensure that capacity limits are controlled
B4. Where Alcohol is being sold under the Premises Licence (i.e. not via a Temporary Event Notice arranged by the Hirer), a Member of or someone approved by the Management Committee will be present. This person will be a Personal Licence Holder (normally the Resident Caretaker), even if the person is not the DPS.
